How to Recognise and Interpret Radiological Signs Commonly Tested in the FRCR Part 2A

Why signs matter so much

Examiners love a named sign. They are neat, memorable and they test whether you can pattern match under pressure. A single well-chosen image with a classic appearance can carry a whole question. If you recognise the sign quickly, you free up time for the harder items later.

The trouble is that signs are easy to muddle. Plenty of them look similar, and the names are not always intuitive. So let us think about how to actually learn them rather than just reading a long list and hoping.

Learn the sign and the meaning together

Here is the mistake most trainees make. They memorise a sign as a picture, tick it off, and move on. Then in the exam a stem asks what the sign implies, or which condition it points to, and they freeze.

Always learn three things at once:

  • What the sign looks like (the visual)
  • The anatomy or pathology that produces it (the mechanism)
  • The differential it suggests (the clinical link)

Take the silhouette sign on a chest radiograph. The picture is loss of a normal border. The mechanism is that two structures of the same density are in contact, so the interface disappears. The link is localisation: loss of the right heart border means right middle lobe, loss of the left hemidiaphragm outline means lower lobe. Learn all three and the question cannot come at you from an angle you have not seen.

Group signs by system

Signs cluster by region, and studying them in groups helps them stick. A few reliable exam favourites to anchor your revision:

Chest

  • Silhouette sign for lobar localisation
  • Air bronchogram pointing to airspace disease
  • Deep sulcus sign for pneumothorax on a supine film
  • Continuous diaphragm sign for pneumomediastinum
  • Golden S sign suggesting a central obstructing mass with collapse

Abdomen

  • Rigler sign and football sign for pneumoperitoneum
  • Coffee bean sign for sigmoid volvulus
  • String sign in Crohn disease
  • Target or sandwich signs on cross sectional bowel imaging

Musculoskeletal

  • Fat pad sign for a subtle elbow effusion
  • Fallen fragment sign in a simple bone cyst
  • Sunburst and Codman triangle for aggressive periosteal reaction

Neuro

  • Empty delta sign for dural venous sinus thrombosis
  • Hummingbird sign for progressive supranuclear palsy
  • Molar tooth sign for Joubert syndrome

You do not need every sign in existence. You need the commonly tested ones understood well.

Do not stop at the buzzword

A sign is a starting point, not an answer. The single best answer format often gives you a description and expects you to choose the best next step, the most likely diagnosis, or the associated complication. So push each sign one step further.

Ask yourself: if I see this, what do I do next? For an empty delta sign, that might be CT or MR venography to confirm and assess extent. For a coffee bean sign, it is decompression and the risk of ischaemia. Training yourself to think past the label is exactly the habit the exam rewards.

Beware the look-alikes

Some signs are traps because they resemble each other or share a name across systems. The target sign appears in intussusception, in some liver lesions and in erythema migrans described clinically. The whirl sign turns up in volvulus and in ovarian torsion. When you meet a sign, note where else that word crops up so a familiar term in an unfamiliar context does not throw you.

Watch the modality too. A sign on ultrasound may have a different cause from a similar looking appearance on CT. Read the stem carefully and confirm which imaging you are actually looking at before you commit.

Make the images do the work

Signs are visual, so passive reading only gets you so far. You have to see many examples. The variation between a textbook diagram and a real, slightly grainy image can be the difference between a confident tick and a guess.

Build a personal image bank as you revise. When you meet a new sign, save a clear example and a borderline one, and write a one line note on the mechanism and the differential. Reviewing these in short bursts is far more effective than one long session.

Try this loop for each sign you meet:

  1. Name it and picture it.
  2. Explain the mechanism in one sentence.
  3. List the main differential or clinical implication.
  4. Decide the next best step.
  5. Test yourself a few days later with a question you have not seen.

Repeat across systems and the signs stop being a memory burden and start being quick wins.
